Bellis brings efficiency and safety to older people’s homes by piloting holistic digital technologies supporting shared care planning and integrated care. The three institutions of the consortium are aligning their expertise to address the Pharaon project’s overall objective and support Europe’s ageing population. Bellis introduces to the Pharaon digital ecosystem hub a state-of-the-art suite of digital tools named BONVITA, which integrates all essential components for conducting a comprehensive geriatric assessment, creating shared care plans, providing effective screening, prevention and management of frail and pre-frail older adults, and offering rehabilitation and quality of life personalized interventions. BONVITA exploits views designed explicitly for care professionals, informal caregivers, and older adults, providing services to support remote rehabilitation through serious games, lifestyle monitoring, medication management, education and training, intelligent planning, recommendations, alerts, smart calendars, intelligent dashboard, reporting and analytics. AI algorithms perform patient stratification, risk prediction and frailty detection.
